LIVE AB Participation Waiver

Baseball and softball activities carry inherent risks including but not limited to being struck by a ball, bat, or equipment; collisions; overuse injury; and other physical injuries. Parent/guardian and player voluntarily accept these risks.

Parent/guardian releases LIVE AB, partner facilities, operators, staff, volunteers, sponsors, and vendors from any and all claims arising from participation, except in cases of gross negligence or willful misconduct.

In the event of injury, parent/guardian authorizes LIVE AB event staff to secure emergency medical treatment. Parent/guardian is responsible for any resulting medical costs.

Parent/guardian grants LIVE AB permission to capture and use photos, video, and audio of the player for event coverage, marketing, sponsor recap, and social media, without additional compensation.

Players are responsible for bringing appropriate protective equipment (helmet, catcher's gear where applicable) and for the safe, respectful use of all facility and event equipment.

All participants agree to follow facility rules, event staff instructions, and safety protocols posted at the venue. Failure to comply may result in removal from the event without refund.

The signer certifies they are the legal parent or guardian of the player and have full authority to execute this waiver on the player's behalf.

Parent/guardian agrees that entering their full legal name and submitting this form constitutes a valid electronic signature, legally equivalent to a handwritten signature.
